Ayurveda is an ancient way of keeping healthy that looks at the whole person, not just parts. It’s a bit like other types of medicine, but it’s special because it’s connected to the Atharva Veda, an old Indian text.
The word “Ayurveda” comes from two words: “Ayu,” which means more than just life, it’s about how our bodies, senses, mind, and soul all fit together, and “Veda,” which means knowledge. So, Ayurveda is all about understanding how everything in us works together for good health.

svasthasya svāsthya rakṣaṇaṃ, āturasya vikāra praśamanaṃ। (Cha Sūtra 30।26)

यानि स्वस्थ व्यक्ति के स्वास्थ्य की रक्षा करना और रोगी को रोगमुक्‍त करना, ये आयुर्वेद के उद्देश्य हैं।

The main purpose of Ayurveda is to help in maintaining the health of a healthy individual and cure the diseases of a patient. These are a brief description of Ayurveda. 

Now, let’s talk about Panchakarma. Panchakarma is a special part of Ayurveda focused on treating diseases. It’s considered very important. But why is it called Panchakarma?

Importance of Panchkarma

A long healthy life has been the wish of every person since antiquity. Diseases are the destroyers of health, wellbeing and life. Ayurveda aims at restoration of health by attaining dharma (pious acts), artha (wealth), kama(desire), moksha(salvation). Those who are seeking a healthy life should attain these four purusharthas by health. Panchakarma plays a vital role in the preservation, maintenance and conservation of health and it makes the biological system to maintain the normalcy, rejuvenate the body and facilitate the desired effect of therapeutic measures that are administered thereafter.

What is Panchkarma?

The word “panchakarma” is made up of two words: “Pancha,” which means five, and “karma,” which refers to various actions or therapies. So, “panchakarma” means five therapies. It involves five main actions along with Purva karma(preparatory) and Paschat Karma (follow-up procedures).
Panchakarma is a special cleansing process in Ayurveda. It helps to purify the body internally using the closest possible route. This process aims to bring the body back to balance and rejuvenate it by restoring the equilibrium of Doshas (Body Energies), Dhatus (Body Tissues), and Malas (Waste Products).

Why Panchkarma?

The Panchakarma treatment eliminates the internal causes of diseases and releases the burden on your whole being. Large quantities of toxic bi-products are formed in the body as a result of numerous factors, including the body’s continuous metabolic processes, wrong body habits, wrong food habits, incompatible combination of food items, suppression of body urges, emotional imbalances, etc. If the elimination system of your body is functioning well, most of these toxins are eliminated naturally. However, if they are mal-functioning, damaging toxins get deposited in various parts of the body. This toxic load results in the impairment of the Doshas, Dhatus (tissues), etc.. Consequently, normal functioning of the body systems are also impaired and our bodies get impacted by the toxicity and general ill health.

How to perform Panchkarma treatments?

Panchakarma is performed in three phases such as Purva karma ( preparatory procedures), Pradhana karma (main procedures), and Paschat karma ( post-operative procedures).

Purva Karma (Pre-Treatment)

Before the actual purgation begins, there is a need to prepare the body with prescribed methods to encourage it to let go off the toxins. Purvakarma comprises two types of preparatory treatments: Snehana (internal and external oleation with natural herbal oils) and Swedana (sudation or steam therapy). During these treatments, the excess doshas are gradually encouraged to move from the limbs to the digestive tract from which they can be eliminated.

Snehana or Sneha pan

This is administration of Medicated Ghee and mainly done as preparation for Panchakarma. The selected ghee is given according to the digestive power of the patient. The quantity is increased gradually on a daily basis and is administered for a maximum of 7 days to make the total system oleated internally. Snehapanam can be used as a curative method and nourishing treatment also.

Swedana or Herbal Steam Bath

This treatment begins with warm herbal oil being gently applied to your whole body. Next you’ll be asked to sit inside a steam chamber – just your head will remain outside. The prescribed herbal steam encourages your body to perspire profusely and release impurities from the skin thereby pacifying the three Doshas. This is a great therapy for helping soothe painful, aching muscles and joints and for giving the skin a special glow.

PradhanaKarma (Main Treatment)

This main phase includes the administration of the fivefold Ayurvedic detoxification procedures. The five Panchakarma treatments deeply purify the body and restore it to perfect harmony.

Vamana(Emetic therapy)
Virechana(Purgation therapy)
Vasthi (Eneme | Rectal Administration of Medications)
– > Niruha Vasthi( Decoction Enema)
– > Anuvasana Vasti (Oil Enema)
Rakthamoksham (Blood-letting treatment)
Nasya(Errhine therapy)

Vamana (Therapeutic Emesis or Vomiting)

Vamana or therapeutic emesis is induced in the body to eliminate the excess mucus caused by the Kapha dosha. Often, this therapy also releases repressed emotions that have been held in the Kapha areas of the lungs and stomach along with the accumulated Dosha. It is highly effective in treating respiratory disorders, depression, insomnia etc.

Virechana (Purgation Therapy)

Virechana refers to the purgation of toxins from the intestines and pitha zones, performed by administering Virechak Aushdhi (purgative medicine or purgative laxatives) in milk or warm water preparation. This therapy is extremely beneficial in eliminating disorders such as dermatitis, chronic fever, heartburn, jaundice, bile related problems such as rashes, skin inflammation, acne, chronic attacks of fever, biliary vomiting, and nausea

Vasthi (Eneme | Rectal Administration of Medications)

Vasti is the most important among Panchakarma due to its multiple effects. Pitta and Kapha are dependent on Vata as it governs their functions. Vasti is not only best for Vata disord- ers it also is equally effective in correcting the morbid Pitta, Kapha and Rakta.

Charaka has considered Vasti therapy as ARDH CHIKITSA (half of the treatment) of all the disease, while some authors consider it as the complete remedy for all the ailments.

In modern medicine, enema is mainly given to re- move the feces from the large intestine while in Ayurveda Vasti is given as a route of administration of the drugs for multiple actions, which acts locally on the large intestine as well as systematically on the body tissue.

The review of the studies done on Basti helped in concluding that Vasti is effective in treating the diseases of all Srotas and especially Vatavaha srotas.

While colon cleansing or hydrotherapy is popular nowadays, it’s important to know that Basti is different and much more specialized. It can’t be compared to these treatments.

During Basti, herbal decoctions and oils are introduced into the colon through the rectum. Unlike some treatments that target specific areas or symptoms, Basti is a specialized procedure aimed at achieving therapeutic goals by expelling body toxins and vitiated doshas by introducing medicated liquids or oils through anus, urethra or vaginal canal.

Here is a list of the most common vastis in Ayurveda along with their various benefits:

- > Snehavasthi (Herbal Oil Enema)

This is an oil enema, and is very useful in removing the accumulated Malas (toxins) and cleansing the colon. It is also the main therapy for all vatha disorders such as constipation, neurological ailments, paralysis, flatulence, lower backache, gout and rheumatism. As the main site of the vatha Dosha is the colon, the vasthi is used to help restore balance. It nourishes and rebuilds the dhatus (body tissues) and strengthens the body’s immune system.

- > Kashayavasthi (Herbal Decoction Enema)

This therapy is normally administered after a course of Sneha Vasthi. This is a cleansing enema where a combination of honey, oil, herbal paste, herbal decoction, etc. is administered as per the patient’s requirement and condition. This therapy eliminates vatha, clears blockages in the body channels or digestive systems and nourishes the body tissues. It is used to treat disorders such as arthritis, constipation, neurological ailments, paralysis, flatulence, lower backache, gout and rheumatism.

Rakthamoksham (Blood-letting treatment)

Impure blood is drained out of the body through various ways such as needle-pricking, venesection, leeching, etc. If you are feeling slightly alarmed by the thought of this treatment, please be assured that it is safe, painless and effective and performed by experts.

Nasyam

Ayurveda describes the nose as the doorway to the brain and the passageway for life force to enter the body. Nasyam is a therapeutic treatment for the nose, throat, sinuses and head. Herbal extracts medicated oils or powders are instilled in accurate dozes into the nostrils to expel blockages. Pre and post preparation is required in this treatment. Nasyam is very effective in cleansing & strengthening the nasal path, sinuses & very effective in curing different types of headache. It is beneficial for conditions such as sinus congestion, sinusitis, allergies, headaches, migraine, epilepsy, tinnitus, hearing loss, impaired vision and lack of concentration or memory loss.

Paschata Karma (Post-Treatment)

Conclusion

Ayurveda recognizes the uniqueness of the individual and the role of dosha imbalances in disease formation. As panchakarma addresses these imbalances, there is no ‘one size fits all’ treatment. The therapies of panchakarma are customized for each patient, depending on their natural balance of doshas or Prakriti, their health status, age, gender, appetite, and strength, as well as the environment and season.
